Stress Worksheet
Use our stress worksheet templates to identify the source of patients’ stress and help them formulate a strategy to achieve a healthier lifestyle.
What is a Stress Worksheet?
A stress worksheet is a document medical practitioners give to their clients to aid them in dealing with the stress they’re feeling. When the practitioner receives a filled-out worksheet, it doubles as a tool and resource they use during their sessions to know more about their client, understand their situation, and devise a plan to help them healthily manage their stress.
Though a stress worksheet serves the purpose mentioned above, no matter who made it, its content and utilization still depend on the referring physician.
If you’re a physician needing a stress worksheet for your practice but don’t want to start from scratch, we’ve got the template just for you. In our stress worksheets, you’ll find the following:
- Ample space where the client lists down what is stressing them out
- A table where the client writes down the corresponding physical, mental/emotional, or behavioral symptoms for each stressor
- An inquiry into what the consequences of their stress may be
- A dedicated section where the client and practitioner can brainstorm for coping strategies they can adopt or an action plan they can carry out after the session

How Does it Work?
Step One. Download the Template
Access and download a copy of our printable stress worksheet template by doing either of the following:
- Clicking the “Use this Template” or “Download Template” button above
- Searching for the “Stress Worksheets” in Carepatron’s template library on our website or app
Step Two. Answer the Worksheet
Give your client a copy of the worksheet so they can write down their answers. Alternatively, you can have them answer the questions verbally at the beginning of your session.
Step Three. Process the Worksheet with the Client
Once you or the client have completed the worksheet, it’s best to process the answers with the client soon after. We will leave it up to you how you will approach what’s written in the worksheet.
However, by the end, we highly recommend that you provide a variety of concrete examples of how to cope with their stress and an actionable plan. That way, they have options and can decide which among them is effective.
Step Four. Store Securely
After the session, don’t forget to store the worksheets and clinical notes, if you have any, in a secure physical location or a HIPAA-compliant EHR like Careapatron.

Benefits
Early Recognition
We’ve provided a section where patients can write down what stresses them out. With proper guidance and the use of our free stress worksheet template, the patient won’t only recognize the source of their stress but other things too, like what they can or can’t control, how they can be empowered to cope, and what they can do to when faced with another stressful situation. Recognizing these early on can help reduce the patient’s risk for health problems like high blood pressure, heart problems, etc.
Empowers the Patient
In certain circumstances, seeing all of the sources of their stress and a list of how to cope laid out in front of them can make your patient feel empowered. The template would be even more effective if you were there to guide them to brainstorm an action plan they can write down and follow beyond the four walls of your clinic.
Streamlines the Process
A ready-made stress worksheet can reduce the number of steps between booking an appointment and conducting a session. To add, the practitioner doesn’t have to worry about creating a document to note down results and storing it for future reference.
