What does each section contain?

The EMS chart method comprises five sections where EMS providers record critical patient data during emergencies. The chart acronym EMS stands for:

• Chief complaint (C): This section captures the primary reason for the EMS response, such as a medical condition, altered mental status, injury, or another emergency.
• History (H): This record details the patient's medical background, including existing conditions, medications, and allergies.
• Assessment (A): Involves a thorough physical examination of the patient, noting vital signs, visible injuries, and other physical findings like respiratory distress.
• Rx or treatment (R): Documents the medical interventions and treatments administered, including first aid, pain management, medication, and other life-saving procedures.
• Transport (T): Details the patient's transfer to a medical facility, including the destination hospital, time of transport, and any changes in the patient’s condition during transit.

An EMS Narrative Template does mostly the same things as the chart EMS format, but the EMS narrative covers things like patient dispatch and arrival times, scene safety upon arrival at the receiving facility, witness statements obtained, family and next of kin contact details, and the actions and plans of the EMS crew handling the patient.
