Set an appointment 

Before starting a fitness program, it's advisable that a patient first consults with their physician. This is not only especially helpful for patients who aren't in optimal health but also for patients who are in rehabilitation, recovery, or a treatment or management plan for their diagnosed disorder.

Personalize the template

Aside from the quantitative data, encourage them to write down qualitative information in the notes section, such as the effect of physical activity on their mood, what they deem the best time to exercise and any thoughts and feelings they have during or after a workout.
