How to use the DASS 21 Scoring:
Step One. Access and Download the Template
Access and download a digital and printable version of the DASS 21 Scoring sheet by doing either of the following:
Step Two. Conduct the DASS-21 Test
Before you interpret the score, you must obtain a score by conducting the test. If you need a template for the Depression, Anxiety, and Stress Scale 21 test, click here or search for DASS 21 in Carepatron's template library.
Step Three. Interpret the Score
After you have a sum of the answers, use the DASS 21 scoring sheet template to help you interpret the score.
Step Four. Fill the Template
So you may have a copy of the results to refer to in the future, remember to fill out the template. Write down the client's total score and corresponding interpretation. Feel free to write down notes you may have, like the next steps, further testing recommendations, etc., in the dedicated space.
Step Five. Securely Store the Template
Since the template carries sensitive information, you must securely store the filled-out template. Store it at a secure physical location or on Carepatron to limit access only to relevant parties.
