What is a Weight Loss Tracking Chart template?

A Weight Loss Tracking Chart is a simple tool for individuals who are working to lose weight to plot their weight loss graphically. This approach can be a great motivator, as your clients will be able to see a visual representation of their progress rather than just a change in numbers on the scale. Additionally, a Weight Loss Tracking Chart can help to monitor how fast weight is being lost and assess whether an individual’s plan needs to be adjusted accordingly.

This resource is made up of a pre-formatted, customizable weight loss chart template, along with a written log for adding weight measurements. It can be tricky to plot small changes, or to read off the exact dates measurements were taken, and so combining a written log with a graphical representation of the data gives the best of both worlds for your clients.

Staying motivated can be a huge factor in weight loss success, and so we have designed this Weight Loss Tracking Chart to help your client feel proud of their achievements, and work hard to continue to stay on track.

This chart won’t just benefit your clients, however, as it can help you to quickly assess your client’s progress and determine any changes that need to be made to help your clients reach their weight loss goals sustainably and healthily.

How does it work?

This printable Weight Loss Tracking Chart template might look a bit daunting, and many of us haven’t had to use graph paper since math class, but once you or your client have set up the axes, it couldn’t be simpler. Just follow these steps to get your client confident with charting their progress.

Add in the client name and start date

The first step is to add in your client’s name and the date they are starting to track their weight loss using this chart.

Set y-axis values (you only need to do this once!)

In the spaces provided on the vertical axis, enter in weight values. Put the starting weight towards the top, and the goal weight towards the bottom. Then enter in weights in-between using whichever units you like.

Enter weight measurement and date of measurement

Once the chart is set up, it’s time to start entering measurements. Each time your client weighs themselves they should enter it into the written log.

Add any extra comments

Your client might have weighed themselves after a large meal, or just want to note down their mood when they weighed themselves. Whatever the additional note, they can add it in the comments section of the written log.

Plot each value on the chart

Using the values in the written log, your client should then plot their weight measurement and the date it was taken on the chart using whichever markers they like- an ‘’x’’ can work well but it is up to them.
