T1017 is the code used for targeted case management services, which means helping clients connect with the right resources and support they need. It covers things like coordinating care, linking to community programs, and following up to make sure everything is in place.

For example, a case manager might work with someone who has a mental health disorder and help them set up counseling, arrange transportation, and apply for housing assistance. All of those coordination activities would fall under T1017.

Each unit of T1017 usually equals 15 minutes of case management service. So, if you spend 45 minutes helping a client, you’d bill three units.
