HCPCS Code J3590: Unclassified biologics

There is no J3590 CPT code. J3590 is an HCPCS Level II drug code. Use a matching CPT injection code (e.g., 96372 or an infusion code) for administration, and J3590 to describe the drug.

A biologic is “unclassified” when no specific HCPCS code exists for the product, formulation, or indication on the date of service. Unclassified drugs (e.g., J3490) are non-biologics without a unique code. Unclassified biologics (J3590) are biologic products lacking a unique code.
